- Employees may present one selection from List A or a combination of one selection from List B and one selection from List C to establish identity and employment authorization. You must bring original document and all document must be unexpired.

- If you are under the age of 18, please sign and take to your school Counselor for them to issue an OFFICIAL work permit. The request for Work Permit takes 2-3 days to process. If you are under the age of 18, but graduated from high school, please bring your diploma. You will not need a work permit if you are over 18 years old.
- Authorization for TB Test
- You are required to show proof of negative TB result within the last 2 years. If you have a negative TB, please bring your immunization records. If you do not have access to this and do not have access to a medical provider, you can have the test performed at US Health Works for $37. Your other option is to go to your own physician. Once TB test is completed bring test results to New Hire Appointment.
